Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant supports a collaborative project to deploy Avident Health's MyNavigator digital platform to enhance care for patients with metastatic breast cancer (mBC). The initiative addresses critical communication gaps between clinic visits by delivering personalized education aligned with NCCN Guidelines for Patients, promoting enrollment in Unite for HER's free supportive services, and equipping informal family caregivers to foster shared decision-making, treatment adherence, and improved quality of life.
Metastatic breast cancer requires complex, ongoing treatment management where adherence, symptom monitoring, and whole-person supportive care are essential. Yet patients and their loved ones often struggle to navigate clinical guidelines and access community-based resources outside traditional appointments. The MyNavigator platform will serve as an interactive, home-based companion that translates complex clinical pathways into accessible modules tailored to each patient's treatment journey. It directly integrates Unite for HER's free services—including nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts. The platform also engages family caregivers in a secure communication loop, providing context, education, and early symptom alerts to support treatment adherence at home. Additionally, it captures patient-reported check-ins and symptom trends to alert clinical teams early, preventing complications and reducing acute care utilization.
Dr. Maen Farha of Avident Health emphasized the importance of the grant: "Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic. By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success." Sue Weldon, Founder and CEO of Unite for HER, added, "Collaborating with Avident Health on the MyNavigator platform enables us to reach patients and their families directly at the exact moment they need guidance, removing barriers to our integrative nutrition, wellness, and emotional support resources."
The project will measure key quality improvement endpoints over the grant period, including improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics. This initiative matters because it represents a tangible step toward bridging the gap between clinical care and daily life for metastatic breast cancer patients, who often face fragmented support. By leveraging digital tools to deliver guideline-based education and integrate supportive services, the partnership aims to empower patients and caregivers, enhance shared decision-making, and ultimately improve outcomes. The involvement of Pfizer and NCCN underscores the growing recognition of digital navigation as a vital component of comprehensive cancer care.
